Pupil Passports & SEN Support Plans

Below you will find sample copies of our Pupil Passports and SEN Support Plans, which provide adults working with children with a clear overview of their strengths and needs and how these can best be supported. They support the tracking and assessment of SMART targets which are reviewed termly with pupil and parent input.

If you have any questions about your child's Pupil Passport or SEN Support Plan, please discuss these with your child's class teacher or the SENCO.
